The Wellness Group Acquires Hyalite Care Academy, Launches The Wellness Care Institute — A 501(c)(3) Setting National Standards in Care Management Education
Bozeman, MT — November 1, 2025 — The Wellness Group today announced the acquisition of Hyalite Care Academy™, a division of Bozeman-based CrossTx Inc., and its transformation into The Wellness Care Institute, a 501(c)(3) nonprofit organization dedicated to advancing education, certification, and workforce development in care coordination and chronic care management.
The rebranded Wellness Care Institute will serve as the nation’s first nonprofit institute focused on establishing standardized education and credentialing for care management professionals — a crucial workforce that currently lacks a unified training framework or certification standard.
A New Chapter in Care Management Education
Building upon Hyalite Care Academy’s legacy of innovation and the nationally recognized HCACM-C™ (Hyalite Care Academy Care Manager Certified) credential, The Wellness Care Institute will expand its mission through public–private collaboration, research partnerships, and enhanced educational programs.

The Institute’s expanded programming will include new certification pathways, continuing education modules, and collaborative training partnerships with healthcare systems, universities, and value-based care organizations. The goal is to ensure that every care manager in the U.S. — from nurses to coordinators to behavioral health specialists — receives consistent, evidence-based training grounded in both compassion and compliance.
Building the Future of Care Coordination
The Wellness Care Institute will maintain and expand the HCACM-C™ certification, developed under the leadership of Faith Jones, MSN, RN, NEA-BC, and Chandra Donnell, two nationally recognized leaders in care management education. Their program’s comprehensive approach integrates the Seven Domains of Care Coordination, CMS regulatory frameworks, and real-world implementation practices.
The new nonprofit structure will enable The Wellness Care Institute to reinvest all proceeds directly into curriculum development, faculty training, and scholarship programs — creating equitable pathways for professionals entering the field of coordinated and chronic care management.
